STRATFORD FAMILY MEDICAL CLINIC
We have a highly skilled, full time, Primary Care Provider who is here to assist with your medical needs within the scope of Family Medicine. Our qualified TEAM also consists of: #2 licensed LVN’s, Certified Medical Assistant, EMS/Basic, Receptionist, Referral Specialist as well as our Practice Manager.
Our Family/ Primary care providers & TEAM specialize in treating a wide range of ailments for patients of all ages. Their focus is to treat the entire patient, from sick to regular well visits by building a relationship with the patient and their family to gain a complete perspective of their patient’s health status.

Scope of primary care services
Our Providers Providers will also determine when further medical evaluation & treatment is medically necessary by a physician within specialized care, such as General Surgery, Dermatology, ENT, Podiatry, OB/GYN, Cardiology…etc! Our TEAM will then gladly provide the referral steps from locating a provider, making an appt. calling your insurance & obtaining an authorization.
REFERRAL & PRIOR AUTHORIZATION
Prior To Visit:
To assist us please let us know your current status as to private pay, employer (not workers comp) or insurance coverage. Please have an understanding of your insurance coverage, co-pay & deductible.
Time of Visit:
Please assist us by providing the following information at the time of your visit:
Rescheduling / Cancellation:
If you need to reschedule or cancel an existing appointment, please contact our office immediately to assure you a quick next available appointment & to allow another patient the availability of your slot.
Tardiness / No Shows:
It is our policy that if you arrive 15 minutes late for an appointment you may be asked to reschedule. We ask that if you are aware that you will be running late that you please consider phoning to make us aware. If you have a scheduled appointment with us and fail to keep it then we will log this alert within your medical history. Upon repeated occurrence’s we will have no choice but to discharge you from our care.
PHONE CALLS & QUESTIONS
Payments of the following are due at the time of service. Full payment for Private Pay Patients, co-pays, and deductibles for Insured Patient’s. If this is a financial hardship, we will work with you to set up & sign a payment agreement. Please not that you will be expected to comply in order to continue your appointments.
Our staff will answer your questions and, if necessary, one of our TEAM will return phone calls at a convenient time during the day as to not interfere with the evaluation and treatment of other patients being seen in our office. We encourage you to please leave a voice message if the phone lines are busy.
Insurance companies and their policies are known to vary greatly in regards to co-pays and what they cover. Please check with your insurance company to determine what they require in order to cover your appointment with us, or our office can gladly determine that for you before we schedule you in.
Every effort is made to accurately estimate our charges through your insurance company, but please be aware that this will only be an estimate. We do however check online with your insurance eligibility & determination department! The final determination of payment will lie with your insurance carrier
Please help us to ensure what your visit cost implies in order that our estimate is as close to your expectations by notifying your Ins carrier as to what is covered, paid & in network.
Our team will make ever effort to file with your insurance that you have registered on file with our clinic. It is important that our communicate with our TEAM if you change Insurance Companies due to either personal choice, age determination, change of employment, loss of employment or State funding.

If your insurance is not listed, you may contact your insurance carrier or our office to check if we are a participating provider.
